Please submit your price proposal and all relevant documents via East Avenue Medical Center on or before November 25, 2020, 3:00 PM with the following requirements:
Eligibility Requirements:
- Updated Business Permit
- Updated SE or DTI Registration
- Updated BIR Certificate of Registration (Form 2303)
- Updated Tax Clearance
- Updated PhilGEPS Certificate
- Updated Audite Financial Statement including Annual Income Tax Return
- Statement of the prospective bidder of all its ongoing government and private contracts, including contracts awarded but not yet started, if any, whether similar or not similar in nature and complexity to the contract to be bid, within five (5) years from the Submission and Opening of Bids
The statement shall include, for each contract, the following:
- name of the contract,
- date of the contract,
- kinds of Goods,
- amount of contract and value of outstanding contracts, and
- date of delivery
- Statement of the bidder's Single Largest Completed Contract (SLCC) similar to the contract to be bid in accordance to ITB Clause 5.4, within five (5) years from the Submission and Opening of Bids, must be at least fifty percent (50%) of the ABC.
The statement shall include, for each contract, the following:- name of the contract,
- date of the contract,
- kinds of Goods,
- amount of contract and value of outstanding contracts,
- date of delivery, and,
- end user's acceptance or official receipt(s) issued for the contract if completed
- Net financial Contracting Capacity or Credit Line Certificate
- Certificate of exclusive and/or authorize distributorship.
Technical Requirements
- Delivery and Completion Schedule: Thirty (30) calendar days
- Completely filled out EAMC Technical Specification Form.
- The Revised Omnibus Sworn Statement executed by the bidder or his duly authorized representative.
iii.a. Duly notarized authority of the signatory:
iii.a.i Secretary's Certificate (i.e. Corporation; Joint Venture; Agreement); or
iii.a.ii Special Power of Attorney (i.e Sole Proprietorship; Partnership)
Additional Requirements:
- Warranty Certificate
Warranty Certificate minimum of ONE (1) year for SERVICE and ONE (1) year for PARTS. The warranty period shall commence from the date of acceptance by the end-user after testing and commissioning.
Financial Requirements:
- Completely filled out EAMC Price Quotation Form. Bidder must fill in the ff: bid price per unit, the amount in words, bidder's specifications, brand and country of origin. Failure to comply shall be the ground for disqualification.
- Price Proposal in company's letterhead
- Bidder should provide a separate Compact Disc (CD-R, CD-RW, DVD-R, DVD-RW, etc) or Universal Serial Bus (USB) containing the Price Quotation List in an excel file. Only one (1) Compact Disc or USB is required to be included in the Original Financial Folder. Bidder shall submit two (2) copies of their proposal in a prescribed color of envelope properly labeled and sealed. The First Copy mark as 'ORIGINAL'; the Second Party marked as 'COPY 1'. All of the two (2) copies shall contain all the Requirements in a folder.
