The section of Nephrology of the Department of Internal Medicine will be accepting applicants who wish to undergo fellowship training beginning April 25, 2019, until June 15, 2019. Applicants will take a qualifying examination covering General Medicine and Nephrology topics on June 22, 2019. Pre Fellowship will commence on July 1, 2019.

- PRC Board Exam Rating (Certified True Copy)
- Certificate of Completion of Residency in Internal Medicine (Photocopy)
- Certificate as Fellow/ Diplomate of PCP
- PSBIM Score
- RITE score for 2 years (if available)
- Certificate of Awards or Distinctions (Academic and Non-Academic) if any
- Certificate of Good Moral Conduct or NBI Clearance
- PSA Birth Certificate
- Certificate of Training/Seminars attended after graduation (If Any)
- Two (2) 2”x2” ID pictures
