Training Program

The East Avenue Medical Center Department of Dermatology is accepting applicants for residency training until September 24, 2018, (Monday) 4 pm at the G/F Dermatology OPD, East Avenue Medical Center.

Please take note that the written exam will be on September 25, 2018 (Tuesday). The final venue will be announced.

Application Procedure and Requirements

Step 1: Application Form for East Avenue Medical Center Residency Training Program will be available at the Professional Education, Training and Research Office (PETRO) located at the 2nd Floor, Main Building of East Avenue Medical Center from 9 am to 5 pm, Monday to Friday.

Step 2: Payment for the PETRO application form will be settled at the EAMC Cashier.

Step 3: Proceed to the Department of Dermatology at the Ground Floor OPD Building, open from 8 am to 5 pm (Monday to Friday) for other requirements needed (including a separate Dermatology Residency Program Application Form) and information regarding the entrance exam.

Step 4: Pay Dermatology examination fee at the Department of Dermatology.

Step 5: Submit the following documents at the Department of Dermatology in a long brown envelope labeled with your full name (use a black permanent marker) on the upper left side and Passport sized picture pasted on the upper right side of the envelope. the envelope contains:

    1. Application form with essay (this form must be secured from the Department of Dermatology)
    2. Three (3) pieces passport sized pictures
    3. Application letters
      (A) Two copies addressed to:
      EMMANUEL M. BUENO, MD, FPCS, FACS
      Medical Center Chief II
      East Avenue Medical Center

      THRU: ALLAN TROY D. BACQUIR, MD, MMHoA, MPM, FPCS
                 
      Head, Professional Education, Training, and Research Office
       (B) Two copies addressed to:
      MA. ANGELA M. LAVADIA, MD, FPDS
      Chairperson, Department of Dermatology
      East Avenue Medical Center

      THRU: ELIZABETH P. PRIETO, MD, FPDS
                  
      Training Officer, Department of Dermatology
                   East Avenue Medical Center

    4. Letters of Recommendation from two (2) medical school professors (original)
    5. Transcript of Records (submit photocopy AND bring the original)
    6. Diploma (submit photocopy AND bring the original
    7. Class Ranking (submit photocopy AND bring the original)
    8. Certificate of Internship (submit photocopy AND bring original)
    9. Board Rating (submit photocopy AND bring the original)
    10. PRC Certificate (submit photocopy AND bring the original)

Step 6: Examination permit will be given by the department upon completion of all requirements and payment.

Note: The following  will be required during the screening process into the residency program:

  • Psychological examination
  • Philippine Medical Association Membership (Good Standing)
  • A copy of the final paper of past researches
